Forms builds a form from the same question types used across Atrium's people apps, with logic, calculations and a records table for what comes back.
Building a form
Add questions — text, choice, number, date, file upload, signature, and more — across as many pages as you need. Logic shows or hides a question or page based on an earlier answer, requires a field conditionally, or ends the form early. Calculated fields total or compute a value from other answers, shown to the respondent or kept hidden.
Sending a section to someone else
Mark a page as an external section, then choose "Send this for completion" to create a link and, optionally, a one-time 6-digit code to pass on separately (read it out, text it, or print it). The other person opens the link, enters the code, fills their section, and you're notified. This is how a template like offboarding gets a manager's part done without giving them a company email account.
Standard or Private
Choose per form: Standard means our servers can read answers, which is what makes routing, search and integrations possible. Private seals answers to your reviewers in the browser before they're sent, except for any field you mark routing (needed so the server can still direct the response). Neither mode is labelled "secure" or "insecure" — pick the one that fits what you're collecting.
Records
Every response lands in a searchable, filterable table with a timeline and an audit trail. Export as CSV, XLSX, or PDF (for printing or saving one response).
Templates
Start from a template such as onboarding, offboarding, an expense claim, an incident report, event registration, or a feedback survey, each with a recommended privacy mode already chosen.
What we can see
See the "What we can see" panel on any form for the exact list of what's server-readable for that form. As a rule: Standard-mode answers are server-readable; Private-mode answers are not, except fields marked routing. Details: What we can and cannot see.
